پیچ خوردگی مچ پا (  Ankle  sprain) یکی از شایع ترین آسیب های ورزشی است ،آسیب به لیگامان های مچ پا بر اساس محل قرار گیری و مکانیسم آسیب به چند گروه تقسیم می شوند .

پیچ خوردگی از نوع Inversion

در این نوع از پیچ خوردگی ها مچ پا به سمت داخل می چرخد و لیگامان های خارجی دچار آسیب می شوند . پیچ خوردگی های نوع Inversion شایع ترین پیچ خوردگی های پا  هستند .

نوع دوم پیچ خوردگی از نوع eversion می باشد در طی این نوع از پیچ خوردگی ها مچ پا به سمت خارج چرخش پیدا می کند ولیگامان های سمت داخلی مچ دچار آسیب می شوند ،که علت این نوع از پیچ خوردگی بیشتر به دلیل آناتومی استخوانی و لیگامانی می باشد . البته این نوع آسیب به نسبت شیوع کمتری دارد ،دلیل آن هم در این است که قوزک خارجی نسبت به قوزک داخلی در سطحی پایین تر قرار گرفته است  و می تواند به عنوان مانعی برای چرخش بیش از اندازه مچ پا به سمت خارج باشد . ولی در صورتی که این نوع  پیچ خوردگی اتفاق بی افتد ،نسبت به نوع قبل نیاز به مدت زمان بیشتری برای ترمیم می باشد .

پیچ خوردگی از نوع syndesmotic

دسته سوم پیچ خوردگی syndesmotic است ،در طی این نوع پیچ خوردگی آسیب جداگانه در قسمت انتهای استخوان های فیبولا و تیبیا که استخوان های تشکیل دهنده ساق پا می باشند ،در قسمتی که مفصل مچ پا را تشکیل می دهند رخ ميدهد. این نوع پیچ خوردگی بسیار سخت به درمان پاسخ می دهد و اغلب ماه ها طول می کشد تا ترمیم حاصل شود و فرد مجبور است تا مدتی طولانی این عضو را بی حرکت نگاه دارد .

انواع کشیدگی تاندون ها

  • گرید یک

کشیدگی تاندون ها ممکن است  تنها  به صورت گرید یک رخ دهد ،یعنی کشیدنی تاندون ها قابل دید نیستند و تنها با استفاده از میکروسکوپ قابل نمایش می باشند .

  • گرید دو

گرید دو یعنی  تاندون ها دچار پارگی شدند ولی پارگی ناکامل صورت گرفته است و ارتباط تاندون ها قطع نشده است .

  • گرید سه

در گرید سه تاندون ها کاملا دچار کشیدگی شده اند ،به این معنی که ارتباط آن ها به طور کامل قطع شده است . در گرید سه پیچ خوردگی های خیلی شدید رخ می دهد .

شدت پیچ خوردگی

در آسیب درجه 1 ،درجاتی از کشیدگی و احتمال پارگی فیبر های لیگامانی رخ می دهد که ممکن است با بی ثباتی خفیف مفصل مچ پا همراه باشد . در آسیب درجه 2 درجاتی از پارگی و جدا شدن فیبر های لیگامانی وجود دارد و مفصل مچ پا دچار بی ثباتی متوسط می باشد . همچنین بیمار درد متوسط تا شدیدی دارد ،تورم و خشکی مفصل هم وجود در این درجه از پیچ خوردگی وجود دارد .

درآسیب درجه 3 ،پارگی کامل لیگامان رخ می دهد و بی ثباتی واضحی در مفصل مچ پا وجود دارد ،این امر ممکن است در مراحل اولیه همراه با درد شدیدی باشد ، اما بعد از گذشت مدت زمانی به دلیل پارگی کامل فیبر های عصبی ممکن است حتی دیگر دردی احساس نشود . آسیب های درجه 1 و 2 با کمک فیزیوتراپی و لیزر  قابل درمان است ولی در آسیب درجه 3 لازم است تا بیمار چند هفته ای بی حرکت باشد تا تورم از بین رود و در نهایت نیاز به جراحی می باشد .

اقدامات لازم در زمان پیچ خوردگی مچ پا

  • اولین قدم در برخورد با این نوع از پیچ خوردگی ها این است که فرد کاملا بی حرکت شود .
  • بعد از تشخیص و اقدامات درمانی لازم است تا پا، در زمان استراحت بالا نگه داشته شود .
